From Pathways to Practice: Pharmacists Bridging Pathophysiology, Treatment, and Therapy Escalation in Pulmonary Arterial Hypertension Management

Although recent treatment advances in pulmonary arterial hypertension (PAH) have improved patient quality of life and extended survival, prognoses remain poor. Pharmacists play a key role in facilitating access to clinically appropriate PAH medications and supporting patients to maximize their benefits and safety. Advancing patient-centered education and interprofessional coordination to support safe and effective medication use continues to present challenges in pharmacist management of patients with PAH. As crucial educators on disease management, pharmacists require up-to-date knowledge on PAH pathophysiology, therapy optimization, and guideline recommendations so they can support patients in understanding their medications, monitor their effects, and adhere to treatment regimens. This educational activity will provide in-depth knowledge of the pathophysiological pathways involved in the development of PAH along with distinct mechanisms targeted by PAH therapies and their role in different clinical scenarios. This activity will demonstrate practical strategies for implementing current recommendations for risk stratification and therapy selection, including when and how to escalate therapy including combination therapy to reach patient-centered treatment goals.

Jared Frost is a Clinical Pharmacy Specialist in cardiology and the PGY2 Cardiology Residency Program Director at Aurora St. Luke's Medical Center in Milwaukee, WI. Dr. Frost earned his Doctor of Pharmacy degree from Drake University and completed both PGY1 and PGY2 Cardiology Pharmacy Residencies at Aurora St. Luke's. In his current role, he oversees medication practices across the cardiac catheterization and electrophysiology labs, cardiovascular surgery, and pulmonary hypertension services. Dr. Frost is an active member of the American College of Cardiology, the National Lipid Association, and the Pharmacy Society of Wisconsin.

Zachary Klick is a board-certified clinical pharmacist specializing in cardiology in Winston-Salem, North Carolina. He earned his PharmD from Duquesne University Mylan School of Pharmacy and completed his PGY1 residency at Carilion Clinic Roanoke Memorial Hospital, followed by a PGY2 cardiology residency at HCA/UTHSC. Dr. Klick has experience in both inpatient and outpatient cardiology, managing patients with advanced heart failure, pulmonary hypertension, and coronary artery disease.
